My suggestions if you're thinking of Milan - don't go for more than 2 days, it's a finance/industrial city and nowhere near as pretty as the rest of Italy. Go just for the Duomo, The Last Supper (for which you need to buy tickets at least 3-4 months in advance or take one of those annoying bus tours around the city) and the shopping (there are great sales in January and June if that fits into your travel).
My favorite city in the north is Venice, but again, you probably don't need more than 2-3 days to see the whole thing.
In Rome, if you're interested in seeing the Sistine Chapel, be aware that it's only open on certain days, and closes at noon or 1pm (unlike the rest of the Vatican Museums). Get there REALLY early, as the line to get in can take several hours (particularly if you choose to go on the once a month "free sunday" - I waited on line from 8am to 11:30am).
I haven't been, but I've heard wonderful things about Positano.
Also, in the north, the alpine lake region (Como, Lugano, Maggiore) is quite exquisite.
If you want particular recommendations, especially for hotels/restaurants/sights in Milan or Rome, shoot me an e-mail and I'll be glad to help.
Ask me anything.
I just got off the phone with a friend, who was telling me about the Amalfi Coast area and Pompeii. I definitely want to get to each of those. She said Naples is full of criminals, and I've heard there are many pickpockets in Rome, but that doesn't affect the experience. Just be careful. She also said summer is awful, hot and crowded.
I'll know more when I get back.
